The type of personal information I collect

I currently collect and process the following information:

  • Brief handwritten notes on each session using just your first name.
  • Assessment forms that include your contact details (name, address, mobile number, email address, GP and next of kin) and your reasons for seeking therapy, which are destroyed after completion of therapy.

How I get the personal information and why I have it

Most of the personal information I process is provided to me directly by you for one of the following reasons:

  • In order to be able to contact you, set up meetings and in case of emergency.
  • In order to provide you with the professional service of Psychotherapy.

I may also receive your personal information indirectly, from the following sources in the following scenarios:

  • A professional psychotherapy organisation or colleague may refer you to me by forwarding your details from which I will contact you to discuss potential therapy with myself.

I use the information that you have given me in order to:

  • Provide Psychotherapy services.
  • To contact you in relation to the services provided.
  • To support, maintain and improve services delivered to you through supervision.
  • Update professional executors and in the case of my death or incapacitation so that they can contact and inform you accordingly.
  • Where risk and safeguarding issues arise that may require contact with social and emergency services.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are:

   (a) Your consent. You are able to remove your consent at any time. You can do this by contacting Neil Turner.
   (b) I have a contractual obligation.
   (c) I have a vital interest.
   (d) I have a legitimate interest.

How I collect your personal information and from where:

  • I receive your email address from you as a result of you contacting me via my website, via you finding me via a web search or via various counselling services directories.
  • An assessment form is emailed to you, which you complete by adding your contact details and reasons for seeking therapy. You then email this back to me. This is then printed out and kept in a secure file until therapy is complete.

The reasons I need to collect and hold your personal information:

  • In order to be able to contact you regarding Psychotherapy services, set up online meetings and in case of emergencies.
  • I hold your personal details after our work is complete for up to two years. This is in case you decide to work with me again and therefore may need to refer back to your initial assessment information.

Instances where I may need to pass on your personal information to a third party:

  • Where I may need to refer you on to another colleague or other services.
  • Whilst in therapy with me your contact details are shared with professional executors who, in the case of my death or incapacitation, will contact you to inform you of the situation.
  • Where risk and safeguarding issues arise that may require contact with social and emergency services.
  • In psychotherapy supervision where your full details are not disclosed unless in an emergency situation.

Your right to withdraw consent

  • You may withdraw consent to me in keeping your personal details at any time by contacting me directly and requesting this. If we are still working together this will mean the termination of our work together as your personal details are necessary for me to provide ongoing Psychotherapy services to you. You also can request that I don’t keep your details on file for any further period after completion of our work.

How I store your personal information

  • Your information is securely stored
  • I keep brief notes after each session for my own purposes, which do not use your full name our any other personal identifiers.
